To obtain any Settlement Benefits from BrassCraft, you must follow the instructions on the Claim Form. All Claim Forms, together with supporting photographs, documentation and any other information, as applicable, must be mailed by first-class United States Mail, postage prepaid to the Claims Administrator at:

To file a claim, print, complete, and mail the fully completed Claim Form below with the appropriate supporting documentation according to the instructions on the Claim Form by the claim filing deadline.
